Patricia (Trish) Moore MSN, RN, CDE is a Clinical Nurse Specialist (CNS) in the care of adults with diabetes and other related chronic diseases. Her experiences include:
• Business owner of Diabetes Care Concept, PC, a private clinical nursing practice in which she taught and coached patients in self-care management of their disease.
• Consultant to physicians and hospitals for diabetes program development and management.
• Educator for Clinical Solutions, LLC worksite program, Wise Health Decisions®, to provide ongoing chronic disease and wellness education to employees and spouses.
• Co-designer of chronic disease and wellness program, materials, and outcome measures for employees with chronic disease.
• Leader in national diabetes organizations. She served on the American Association of Diabetes Educators Chapter Presidents' Council and on the National Certification Board of Diabetes Educators (NCBDE) and Item Construction Committee.
• Clinical Research Nurse for Indiana University Medical Group where she was involved in the implementation and monitoring of geriatric research projects at the Roudebush Veteran's Administration Medical Center.
• Contributing Author to multiple nursing books and research articles.
• Coordinator of diabetes care for the Diabetes & Pregnancy Clinic at the Indiana University School of Medicine, Diabetes Research and Training Center.
• Director of Diabetes Education and Resource Center and Diabetes CNS across the continuum, Columbus Regional Hospital, Columbus, IN.
